Case management minimizes the emotional pressures families and/or individuals experience which can often result when embarking on the addiction and mental health recovery journey. Case management provides education and professional accountability for individuals affected by addiction and mental health challenges. It is a commitment to rebuild healthy relationships and aid in the recovery process.

Using a two-step approach based on clinical need and personalized health care, our case managers customize a recovery treatment plan comprised of weekly consultation, recovery coaching which aligns with the clinical needs of the individual.

As needed, our case managers will mediate communication between our clients, their families, and all care professionals involved in the recovery process.

Our holistic approach to care ensures our case managers are continually evaluating and refining the prescribed treatment plan based on the individual.

Rooted in Recovery works with a number of monitoring tools in order to promote recovery accountability.

Rooted In Recovery clients are individuals struggling with drug and alcohol addiction and/or mental health challenges who require assessment and referral services and need solutions to address a variety of ongoing crisis interventions. Our clients are also families of loved ones who have expressed a desire to learn new recovery coping skills and wish to provide a healthy support system for the individuals struggling with alcohol, drug and substance abuse addictions.

The addiction recovery process is different for everyone and involves a combination of lifestyle changes, professional help, and support from family and friends. It can include detoxification, residential treatment, therapy sessions with a mental health professional or counselor, 12-step programs and ongoing support to maintain sobriety.

Case Management is designed to benefit anyone who is entering a drug, alcohol or substance abuse treatment program or navigating alternative options at the time of the intervention. Our programs are also designed to support individuals completing substance abuse and mental health treatment programs and families requiring professional support.

During the first 30 days of service, our case managers carefully select and develop treatment plans and recommendations for our clients. After this period, a review of the client’s needs and recommended support services is done, at which point updates to the recovery plan are provided.
